About the Toolkit
The Colorado Safe Transfers Coalition Implementation Toolkit was developed to support a more connected, collaborative, and relationship-centered perinatal care system across Colorado. Grounded in the understanding that safe birth care exists across multiple settings and levels of care, these tools are designed to strengthen communication, clarify transfer processes, and support respectful coordination between community birth providers, hospitals, EMS teams, and families.
By creating shared frameworks, educational resources, and adaptable transfer tools, the toolkit aims to foster greater trust, continuity, and interdisciplinary collaboration during some of the most important moments in perinatal and newborn care.

Why Collaborative Transfers Matter
Most planned community births progress safely and normally. However, there are times when consultation, transfer, or a higher level of care becomes necessary. During these moments, families and providers may encounter differences in care models, communication styles, institutional processes, or expectations that can create stress, misunderstanding, or fragmentation during transfer experiences.
